An accident happened about twenty years ago or so and was reported in Aviation Safety.  It's one that you never forget because the perfectly running Cessna slammed into mountainous terrain while basically flying straight and level.  The female occupant ended up with her head somewhere under the pilot side instrument panel and the pilot's clothing was in sufficient disarray that the diagnosis was fairly clear.  You just have to wonder after all these years if he was able to achieve the happy ending before the lights went out.  No report in the article about body fluids so we are left to ponder.  

Darwin awards are always there for the winning.

Sounds un-professional and a lack of good judgement in this model of high performance aircraft doing 5 miles a minute. You can't safely fly if you are not mentally engaged.

I recently completed my annual recurrency with LOBO, but I didn't discuss this crew resource management issue with my instructor.
