As a note, I was sorry to hear of your misfortune.  A few years ago, we flew the Legacy together for a demo, realized we had a common bond through our jet warbird activity, and just the plain love of 6 degrees of freedom.  I herald your reply as something that we should all do, regardless of our title, credentials, or numbers of A/C flown.  The bottom line.. "So, keep your heads out, be aware when routines are broken and use that checklist."  When situational awareness brakes down, things do happen, many times not to our liking or our ego's.
