The couple exhausts that I've welded were tacked on the outside, and welded on the inside, though I've never seen a commercial system done that way.  My primary reason for welding the inside is that it's a MUCH easier weld for me to make, and if it's ugly, no one will see it :-)  The downside is that everyone looks at the tacks on the outside, and says "You forgot to finish welding this." :-)
