October 8, 1996, a Lancair IP crashed at Camarillo , CA after loss of power with a fuel selector modified to include a both position. NTSB investigation determined that the plane was being operated with fuel in the left tank and no fuel in the right tank. The modified fuel selector spindle allowed air from the right tank to bypass the internal o ring and enter the fuel stream causing a lean condtion.
