When someone has any idea as to why the prop came off, please let us know.

Was it the safety wire? I can't imagine.
Crankshaft broke?
Crank flange cracked? To me, the most likely.
Prop broke?

Back in 2005, on a gear up landing, after inspection, my crank flange was found to have a 180° crack. However, this was not just a normal gear up. I landed. The landing felt hard. I took off again, came back around and landed a second time with the gear up. The plane climbed faster then usual with the Hartzell tips bent.
