Re the window bulletin, I'm not certain, but I believe the bulletin came out
after this window was already installed.  Upon removal and inspection of the
remaining pieces, it looks like the crack started at one of the holes
drilled for the door latch mounting studs.  As I mentioned earlier, the
plane had well over 300 hours (closer to 400 actually) on it at the time of
the incident, so it took quite some time to fail.
