Gentlemen:

As an update on the crash of Lancair IVP N424E which took the life of Tony Durizzi in Flagstaff AZ on June 16th.   The investigation has shown that in addition to the right wing not being built correctly and missing the fuel vent for the D section of the wing at rib 141, the slosh door in the left wing was not functioning due to resin which leaked onto the door freezing it shut.

The theroy is that when Tony found himself unable to draw fuel from the right wing because of the suction created by the lack of proper venting in D section, he switched to the left tank.   This left tank which had the inoperative slosh door probably came un-ported causing loss of fuel flow.

The NTSB and FAA have both launched investigations into the activities of John Kerner who in additon to being the professional builder who completed N424E was also the builder who worked on two other Lancairs which crashed.   In at least one of these crashes fuel system problems were a factor.

Any thoughts, ideas or advice is welcome.
