I would say that was an assembly problem. notice how clean the rotors are. I bet that motor never started

I thought when I had the small triangular part of the apex seal caught between rotor and housing was a very rare and unusal failure mode.  But, apparently there are more dramatic ones - here is a photo of one with the corner seal embedded in the face of the rotor - How did it get there??
Impressive!  Yes, the rotor housing that shoved it into the rotor was trashed as well.
