Do I make them and then firesleeve, or does the firesleeve have to be on before the fitting goes on?” The firesleeve goes on after you make the line. Firesleeve (typically a fiberglass woven fabric) has a hi-temp silicone coating. The ends of the firesleeve should be dipped in liquid silicone but you can get by with just a little hi-temp (normally red) silicone applied to the cut end. This is to prevent the fabric from acting like a wick and absorbing oil etc. After the hose is made, the firesleeve (cut to length and the ends sealed) is slid over the hose and then secured in place at each end with stainless steel bands (like ty-wraps). You can also substitute a few wraps of safety wire. The other option is to get your hoses made by a hose shop. They'll come professionaly finished and pressure tested. You specify the size, length, and fittings. This is what I do when I install a new engine. "Do I have to worry about fireproofing the metal bulkhead fittings for the fuel lines where it penetrates the firewall? (I wouldn't think so.)" I can’t say I’ve ever seen firesleeve on bulkhead fittings, HOWEVER firesleeve does serve a second and important purpose ......... it acts to insulate the fuel in the line from the high heat of the engine compartment which helps to eliminate vaporization in the fuel lines.
