I found this newsletter from Australia that told an interesting story about a Mooney glass retrofit and the subsequent chain of events during the test flight.  Somehow the accident chain got broken before the airplane did.  It is a great story about brain overload and dealing with unknowns in a deteriorating situation.  The story is applicable to all of us making changes to our systems.
