I spent a lot of time with tapered roller bearings. Even with a full machine shop, we never tried to make such a spacer, and to make one without a lathe would be really tough. The length of the spacer would have to be just right within .0005" or so. What we did instead was to peen the shaft where the inner race rides. Usually, even if the race had spun before (but not worn too much), about 50 hits the width and circumference of the race with a center punch would allow the bearing to be adjustable without spinning. When the pre-load is correct, the wheel will turn by hand easily without any slop. Too tight is just as bad as too loose.
