If I can correctly recall the minimum is:

Year Manufactured: Year Inspected by the FAA REP from MIDO Serial Number:
You choose any number" I choose the serial number of the kit
Make & Model:
I cannot recall if you must have the N number on the tag. Perhaps someone
else can chime in here on this.

Don't buy those ugly data plates from aircraft spruce.
The information can be engraved, stamped or scratched on a stainless sheet
about 1 by 2 inches and rivited or siliconed on the fuselage side just under
the H stab leading edge. The regs say it must be seen from the underside of
the craft. I originally glued it on the H stab underside but the judges at
Arlington missed it in its first month of infancy and I was docked for it.
So it was moved under the leading edge of the H stab and glued onto the
fuselage. Technically you could glue it on the bottom of the fuselage.
