In my case, it was the source of the failure. Upon initial disassembly, the frayed washer and small pieces of it in the cylinder were evident. The O rings which were supplied in the repair kit did not need to be replaced. Someone mentioned that the part number and procurement source for this washer are known. I must have missed it. Can you please repeat that information. I still have the failed cylinder in need of the washer. I purchased a new cylinder to replace it so I get back to flying.
