It is a Laminova water-to-oil heat exchanger.  I bought mine used off ebay.  I Googled "laminova" and came up with what looks like a good source at http://www.racerpartswholesale.com/category/Mocal_Heat_Exchangers.

The main distributor is Bat Inc. (http://www.batinc.net/files/laminova.pdf)

You can order it with whatever size fittings you're using.
