The item # is 2E158, the cost $13.78. You will also need a utility box to affix it. I use it to drive a two bulb setup. Either bulb will be able to reach the desired temperature on it's own in case one burns out. Of course both work much quicker. I also installed a small 120V fan to circulate the air. Make sure the thermostat is as far as possible from the heat source. I hope this helps.
