Yes, ICAO and US customs require 12 inch numbers
BUT they can be temporary, ie: tape.

get by with the old ELT,
Yes, Bahamas is not yet requiring the 406
ELT.
and is a radio
station license required?
Technically yes. A station license is required for
any flight out of the US. Practically speaking, I've never heard of it being
checked.
Has anyone stayed at Sammy T's on Cat Island and how did you like it?
Haven't been to Sammy T's but I like Cat Island.
