In regard to the inner handles working & not the outer handles.  I've had the
same problem before.  What I found was that there wasn't enough clearance for
the cap head screw that links the outer & inner handles.  What I had found
was that the screw was rubbing on the slot of the silver handle, I had to use
my dremel to make enough clearance.. works great now.. hope this helps.
