Your concerns about fuel management are well received.
However, Tony Durizzi and I had conversations about these same issues less
than six hours before he died.
He fully understood that one could flood out a TCM engine by inappropriate
use of the fuel boost pump. He had experimented with both the high and low
boost positions at both high altitude and low altitude.
I believe there is little doubt he used the high boost pump in an effort to
find the fuel left in the otherwise dry or nearly dry tanks.
Given the fact that the boost pump continued to run during the impact
period, and maybe afterwards, it is not much of a surprise that there was
fuel in the spider, as the engine probably had some unusable fuel that
suddenly was wildly re-located so as to become useable by the boost pump
before it quit turning.
I believe, in the end, the evidence will be persuasive that Tony left KADH
rationally believing he had 78 gallons of useable fuel, when he only had
between 62 and 72 gallons of useable fuel. It is possible, given some
scenarios, that he vented some fuel during the climb to altitude through the
over flow vents.
